Wal-Mart has been fined 45-thousand dollars over a foot injury suffered by a customer at the Mapleview Drive store in the fall of 2008. A judge found the company had violated the Occupational Health and Safety Act by not having a signaller working with a forklift operator. The forklift struck the customer while the operator was placing skids of water at the main entrance.
